If you would like to have a product remade, please complete the request form below. To expedite the process, ensure that all the mandatory fields are filled out before submitting the form.
A team member will review your request and contact your office if we have any questions. If your request is eligible for a remake, we will remake the product at no charge. If it is not eligible for a no charge remake, we will create a new case for the remake. Please refer to our Remake Policy for more information.
